Ability adjust the font size of the Quoteform title/subtitle

Currently the subtitle bleeds into the title of the Quoteform.

Change the subtitle to be a dark grey instead of black, with a less bold font in a slightly smaller size.

Add a "description" below the subtitle for the QuoteForm to allow users to add additional context items.
